Basic Life and Accidental Death and Dismemberment

Staff Plan

Employers select from one of the following four options:

Full coverage is available to employees up to their 65th birthday. Between the ages of 65 and 70, the maximum coverage is reduced to $50,000.

Executive Plan 

Adds the option of a Life and AD&D benefit up to five times annual salary, to a maximum of $500,000 each. Please ask for dismemberment coverage details.

 

Dependent Life

Staff & Executive Plan

Spouses are covered for $10,000 and each dependent child for $5,000. Coverage is in effect until the insured employee attains age 70.

 

Optional Life & Accidental Death and Dismemberment

Staff & Executive Plan

In addition to their Basic Life and AD&D coverage, employees can apply for up to $500,000 Optional Life and $300,000 Optional AD&D insurance. Optional Life coverage is subject to medical approval by the insurer.

Spousal and Dependent Child(ren) coverage is also available under the Optional Life and a Family Plan under the Optional AD&D.

 

Long Term Disabilty

Staff & Executive Plan

Employers can offer their employees the security of insured earnings in the event of a long-term disabling health condition. Upon approval of an LTD claim, benefits come into effect 121 days after the employee ceases to be able to work.

LTD benefit is either 77% monthly earnings or 66 2/3% monthly earnings.  Benefit is taxable if the Employer pays the premium, and non-taxable if the employee pays any or all of the premium.

 

Short Term Disabilty

Staff & Executive Plan

Employers can offer their employees the security of insured earnings in the event of a short-term disabling health condition. For an approved claim, benefits come into effect on the first day if due to an accident and on the 8th day if due to an illness. Benefit payments will continue on a weekly basis for 120 days or until recovery, whichever occurs first.

Benefit is 70% weekly earnings.  Benefit is taxable if the Employer pays the premium, and non-taxable if the employee pays any or all of the premium.

 

Extended Health Care

There are two packaged options available within this program and three separate additional optional benefits to choose.

Option 1

Staff Plan

  • 100% reimbursement for all coverage

  • $25 annual deductible per person per family

  • $225 vision care benefit per person every two years

  • Physician Referral coverage for treatment not available in Canada

  • Semi-private and private hospital accommodation

  • Medical Practitioners reimbursement at $400 per person per calendar year

  • Worldwide emergency medical coverage

  • Unlimited overall maximum

  • Coverage available to age 70

Executive Plan

Similar to the Staff Plan except:

No annual deductible; vision care benefit is $250 per person every two years

Option 2

Staff & Executive Plan

The same as Option 1 Staff Plan except coverage is at 80% reimbursement (except emergency out-of-country medical expenses which are reimbursed at 100%).

Optional Benefits

 

DIRECT PAY DRUG CARD: enables prescription drug claims to be paid at point of sale, eliminating the need to submit claims for reimbursement.

EYE EXAMINATIONS: reimbursement of the cost of Eye Examinations to a maximum of $75 per person every two calendar years. Available in British Columbia only.

MEDICAL REFERRAL TRANSPORTATION BENEFIT: reimbursement of specified costs associated with travel and accommodation for employees and/or their dependents who, with physician or dentist referral, are required to travel more than 100km from their place of residence to obtain medical or dental care.

Basic Services: oral examinations, cleaning, scaling, flouride treatment, consultations, x-rays, extractions, fillings, endodontic and periodontic treatments.  No annual or lifetime maximums.

Major Restorative: crowns, bridges, onlays and/or overlays, dentures.  No annual or lifetime maximums.

Orthodontics: corrections for malocclusions with a maximum lifetime benefit of $3000 per person.

 

Executive Plan

This adds the option of 100% coverage for all Dental procedures. No monthly premium is paid for this option, however, your organization is charged the total cost of all dental claims plus an administration fee.
